Privacy Consent
There is now a legal requirement that we gain your consent to collect and use personal information about you. Please read the following carefully and sign the declaration if you consent to Ortho Ski collecting this information. In order to properly assess, diagnose and treat you, Ortho Ski needs to collect some personal and medical information from you. This information may also be used for:
The administrative purposes of running the practice.
Billing, either directly or through a third party (Health Funds).
Use within the organisation, when passing information to other clinical staff for your ongoing treatment and care.
Disclosure of treatment and medical information to you or other clinical treatment providers. In the case of an insurance or compensation claim, it may be necessary to collect and/or disclose information that affects your treatment and return to work. The provision of your high quality treatment may require the sharing of information about your condition, treatment and in some cases third-party claim status may be shared with other treatment providers, insurers, solicitors and employers.
Consent to Treatment
It is part of our duty that all podiatrists and other allied health practitioners inform you of any possible risks associated with professional treatment techniques utilised during a hands-on treatment session. Some therapy techniques such as therapeutic massage, joint manipulations, traction/distraction or mobilisations have a minute risk of causing injury. Injury may be caused to structures including, but not limited to; nerves, bones, muscles, ligaments, intervertebral discs or arteries. The use of tape or massage mediums may cause irritations or allergic reactions on the skin of some individuals. Where possible, hypoallergenic tapes and massage mediums will be used to further reduce the risk of this occurring. Superficial Dry Needling and the use of acupuncture needles, along with the aforementioned techniques can occasionally cause localised swelling, bruising, or transitory increased in the levels or distribution of pain or other symptoms. If you having a General Treatment involving the cutting of toe nails and removal of callused skin, sharp tools are used which can result in cuts to the surface of the skin. In rare instances, infections can occur if these cuts are not managed properly. You may choose to consent or refuse any form of treatment for any reason including religious or personal reasons. Once you have given consent, you may withdraw that consent at any time.
Covid19 Policy Winter 2020
The following steps will be taken:
Face masks are to be worn for the duration of appointments by both client and clinician, as appointments last for well-over the risk period of 15 minutes. Clients may bring their own mask or purchase a disposable one from Ortho Ski for $3.
If anyone is showing signs of infection as described by the Australian Department of Health, they will be rescheduled and not treated that day.
Symptoms include:
– fever
– respiratory symptoms
> coughing
> sore throat
> shortness of breath– Other symptoms can include runny nose, headache, muscle or joint pains, nausea, diarrhoea, vomiting, loss of sense of smell, altered sense of taste, loss of appetite and fatigue.
Only two clients may be in the on-mountain clinics/shops at any time, therefore appointments are required to ensure 4sqm per person
Hands should be cleansed by all clients with alcohol-rub hand sanitiser prior to entering clinic/shop
Clinicians will follow the 5 moments of hand hygiene . Alcohol hand rub can be used a maximum of 5 times in a row before soap and water washing is required. Soap and water wash must be done between all clients.
Clinic layout will provide maximum possible space between clients – more than 1.5m between clients.
Appropriate PPE is to be used by clinicians. It is not to be worn outside of the treatment areas (eg. remove prior to going into workshop – exception of face mask)
Clients’ feet and lower legs are to be cleansed with chlorhex and placed in a new, clean, sock prior to trying boots on. New socks will be issued as part of the regular assessment fee - no extra charge.
All surfaces and equipment will be cleansed and disinfected between clients with a 2-step clean
Declaration
I have read the above information and understand the reasons for collecting information and the ways in which this information might be used. I understand that it is my choice what information I provide but that withholding or falsifying information might be detrimental to my podiatry treatment. I consent to allow Ortho Ski to collect further information related to my podiatric treatment, from other sources as necessary, including x-ray reports, medical reports etc. I am aware that I can access my personal and treatment information on request and, if necessary, correct information I believe to be inaccurate. I understand that if, in exceptional circumstances, access is denied for legitimate purposes that the reasons for this and possible remedies will be made available to me. I understand that Ortho Ski must obtain additional consent if the information collected is to be used in ways other than those described above. I understand that I am financially responsible for any balance due on my account.
I understand that if I have any questions or concerns regarding my treatment I should discuss them with my Ortho Ski health provider before signing this disclaimer. I consent to follow the extra precautions in place for risk reduction of COVID19 transmission. The information I have provided is completely correct and I give my consent to treatment. If you are under 18, you must have your parent or guardian consent to treatment on your behalf.
